Kvartaalsed veremarkerid, igasugu eriarstide rutiinsed kontrollid, keha m&#245;&#245;tmine eri seadmetega, toitumise ja liikumise teemal enda harimine ning muidugi poole &#245;pitust mitterakendamine on pea igap&#228;evased tegevused. Laiemalt on mul tugev usk ennetavasse meditsiini ehk parem natuke enda p&#228;rast &#252;le stressata, kui tulevikus saada hoobilt m&#245;ni suurem tervisemure kaela.</p><p>&#220;ks mu 2024. aasta l&#245;pus tehtud uusaastalubadusi oli v&#245;imalikult palju oma igap&#228;evategevusi teha kas koost&#246;&#246;s AI-ga v&#245;i delegeerida &#252;ldse AI-le. Kuskilt hakkas idanema m&#245;te, et ma saaks igasugu tervisek&#252;simusi arutada<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-1" href="#footnote-1" target="_self">1</a> ju ChatGPT-ga, sest ei taha oma arstidest s&#245;pru v&#245;i meditsiiniharidusega pereliikmeid iga v&#228;ikese k&#252;simusega t&#252;litada. Seega uusaastalubadustesse lisasin endale &#252;lesandeks luua &#8220;DoktorGPT&#8221;.</p><h2>DoktorGPT &#252;lesseadmine</h2><p>Mu esmane plaan oli ChatGPT-d kasutada oma trennieesm&#228;rkide t&#228;itmiseks (a&#8217;la kuidas rasvaprotsent alandada x%-ni) ning veren&#228;itajate t&#228;iendavaks anal&#252;&#252;siks ja soovitusteks ehk teisis&#245;nu, kuidas k&#245;ik n&#228;itajad optimaalseks saada (n&#228;iteks HDL kolesterool).</p><p>Kui ma teemasse s&#252;venema hakkasin, tuli &#252;sna kiirelt v&#228;lja, et kogu tervise teema tuleb ChatGPT-s lahendada nn <a href="https://help.openai.com/en/articles/10169521-projects-in-chatgpt">projekti funktsiooniga</a>, mis tuli just 2024. a l&#245;pus v&#228;lja. Kasutan siit edasi ChatGPT, DoktorGPT, AI v&#245;i &#8220;tema&#8221; vaheldumisi. Pika suhtlemise peale on AI-le &#8220;see&#8221; &#252;tlemine kuidagi imelik, eriti veel, kui suhtled temaga vahepeal h&#228;&#228;lega.</p><p>Projektide puhul meeldis mulle esiteks idee, et hoian kogu tervisega seotud info (vestlused, failid) &#252;hes kohas koos (mu &#245;rn OCD l&#245;i v&#228;lja), aga teiseks on projektil spetsiifilised omadused, mis just sedalaadi arutluse jaoks sobivad:</p><ul><li><p><strong>m&#228;lu</strong> ehk AI omab m&#228;lu antud projekti sees, m&#228;letades nii projekti kui ka projekti vestluste failides olevat infot ning oskab seda refereerida. Selleks palusin, et teeks selguse huvides &#252;levaatliku tabeli, kus on k&#245;ik lisandid koos kogustega sees ning kalendri jaoks ka (.ics) kalendrikutse, mis tuletaks x kuu p&#228;rast meelde markereid kontrollida ja vajadusel toidulisand l&#245;petada.</p><p>Paratamatult k&#252;sib keegi n&#252;&#252;d: aga Google&#8217;ist oled kuulnud? Teoorias saab k&#245;iki neid teemasid ise googeldades ning Reddit&#8217;is, Youtube&#8217;is jms <em>oma peaga m&#245;eldes</em> ka lahendatud, aga see v&#245;tab kordades rohkem aega. </p><p>K&#245;ike seda tehes avastasin veel huvitava n&#252;ansi t&#228;iendava motivatsiooni n&#228;ol. Kuidas see tarkus oligi: pane endale eesm&#228;rgid ja r&#228;&#228;gi neist teistele, et sul oleks kellegi ees vastutus. Tuleb v&#228;lja, et see teine v&#245;ib ka AI olla. Ja tulebki teatav surve ning hasart peale, kui j&#228;lle uued andmed sisse s&#246;&#246;dad (rasvaprotsent, VO2max jm) ning AI kokkuv&#245;tet tehes mainib, et ma olengi mingid eesm&#228;rgid t&#228;itnud.</p><h2>DoktorGPT-ga igap&#228;evane suhtlus gastriidi raviks</h2><p><em>Krooniline antraalne atroofiline gastriit</em> olid gastroenteroloogi s&#245;nad mulle peale (&#228;&#228;rmiselt ebameeldiva) gastroskoopia tulemustega tutvumist. See on t&#228;pselt nii t&#252;likas nagu see lugedes tundub. Kuidas sellise jama endale kaela, st k&#245;htu, saab ei olegi siin nii oluline (klassikaline kombinatsioon helikobakterist, NSAID-idest, stressist jm), vaid m&#245;tlesin siin oma teekonda kirjeldada koos DoktorGPT-ga.</p><p>Kuigi krooniline gastriit diagnoositi mul juba aasta varem, oli see ligi aasta rahulik ning l&#245;i &#228;gedalt v&#228;lja j&#228;lle kevadel (sestap krooniline). Selleks ajaks olin juba AI-ga omajagu terviseteemasid arutanud ning k&#252;sisin temalt <em>third opinion</em>&#8216;i, sest ilmselgelt tasub nii olulise diagnoosi puhul v&#245;tta ka <em>second opinion</em> teiselt gastroenteroloogilt.</p><p>Ravi puhul oli suur murekoht minule, et kuigi see on pealtn&#228;ha lihtne esomeprasooli (nt Nexium) ravikuur, on see vaid j&#228;&#228;m&#228;e tipp. Arst kirjutas k&#252;ll retsepti ning andis &#252;lds&#245;nalised soovitused stiilis &#228;ra joo kohvi ega alkoholi ning piira rasvast ja v&#252;rtsikat toitu (mis on legitiimsed soovitused), siis praktikas oli mul pikk periood, kus ma ei saanud p&#245;him&#245;tteliselt midagi s&#252;&#252;a, sest valud olid halvavad, eriti &#246;&#246;siti. Ruum sai otsa, nagu &#252;ks poliitik kunagi &#252;tles.</p><p>Ideaalses maailmas oleks k&#245;ik vastused mulle andnud gastroeneteroloog ja m&#245;ni kogenud toitumisterapeut, aga <strong>k&#252;simuste vol&#252;&#252;m oli nii suur</strong>, et praktikas oleks olnud vaja m&#245;lemaga konsulteerida mitu korda p&#228;evas. &#220;ks veebik&#245;ne v&#245;i visiit arsti juurde on 80-120&#8364;, kohati v&#228;ga pika ootamisajaga.</p><p>J&#228;tkasin siis AI-ga konsulteerides raviga (esomeprasool, Gastrotuss, mis on t&#228;iesti super nimi, jt Eestis saadavalolevad ravimid), katsetasin igatpidi erinevate toitude &#228;raj&#228;tmisega (nn <em>elimination diet</em>) ja ka  eksootilisemate toidulisanditega: DGL, Jaapanis v&#228;lja t&#246;&#246;tatud zinc-carnosine, L-glutamine jne, mille k&#245;igi puhul on andmed sellised, et need v&#245;ivad aidata, aga kindlaid uuringutulemusi ei ole. </p><p>K&#245;ige <strong>olulisem AI panus</strong> oli siin t&#228;helepanu juhtimine, et esomeprasooli ei tasu p&#228;evapealt l&#245;petada, sest v&#245;ib tekkida &#252;lehappesus, mis on megavalus. Ehk ravimilt tuleb alla tulla j&#228;rk-j&#228;rgult. Gastroenteroloogi s&#245;nul v&#245;ib see toimida, <strong>aga Eestis seda ravimit nagu ka paljusid teisi ravimeid lihtsalt ei ole</strong>. Sest meie turg on lihtsalt liiga v&#228;ike. Uurisin AI-lt, kus ma seda ravimit saan ning tuli v&#228;lja, et Soomes on see m&#252;&#252;gil kui Antepsin. Mis teeb neid eriliseks ja milles ChatGPT loojad n&#228;evad k&#245;va konkurentsieelist (nn <em>moat</em>) on seesama <strong>m&#228;lu</strong>. Ehk mida rohkem andmeid ma oma AI-le s&#246;&#246;dan, seda paremini ta &#245;pib mind tundma, seda rohkem seoseid ta n&#228;eb, seda paremaid vastuseid ta oskab anda.</p><p>M&#245;eldud tehtud ja s&#246;&#246;tsin DoktorGPT-le veel nii palju andmeid sisse, kui ma sain: <strong>geenitestid</strong>, vaimse tervise m&#228;rkmed jm. &#252;ks lahendamata p&#245;himuresid nn <strong>hallutsinatsioonide n&#228;gemine</strong> ehk teisis&#245;nu ajab AI vahepeal jama ning kujutab asju ette. Minu kogemusel n&#228;eb hetkel AI hallukaid k&#245;ige rohkem, kui palun tal enda eest tooteid otsida. </p><p>N&#228;iteks andsin talle ette paar eesti e-poodi ja &#252;tlesin, et otsigu mulle sobiv toidulisand ja juuksepalsam, mis minu k&#245;iki tervisen&#228;itajaid jm eelistusi k&#245;ige paremini v&#245;iks sobida. Ja ta pakkus v&#228;lja mitu head varianti ning ka paar varianti ... mida &#252;ldse olemas ei olnudki. Sestap ka mu juhis DoktorGPT-le, et oleks konkreetne, tooks teaduslikke viiteid jne.</p><h2>Mis rolli AI v&#245;iks tervishoius laiemalt m&#228;ngida?</h2><blockquote><p>&#8220;M&#245;te muutub huvitavaks siis, kui sa hakkad kartma seda l&#245;puni l&#228;bi m&#245;elda.&#8221; &#8212; Nassim Nicholas Taleb, Prokrustese s&#228;ng</p></blockquote><p>Oma DoktorGPT kogemuse ja valdkonna trendide p&#245;hjal ei imestaks, kui mingi hetk tulevikus muutub sedasorti AI nii tavaliseks tervishoius, et <strong>tervishoiuasutustel tekib juriidiline kohustus AI-lt </strong><em><strong>second opinion</strong></em><strong>&#8216;it k&#252;sida</strong> enne l&#245;pliku diagnoosi tegemist. Ilmselt j&#228;llegi USA-s enne, sest seal on tervishoiu ja kohtus&#252;steem erinev meie omast ning v&#245;iks soosida sellist l&#228;henemist.</p><p>AI kasutamine on arstide seas tundub juba levinud olevat, v&#228;idetavalt on <a href="https://www.forbes.com/sites/amyfeldman/2025/07/15/this-ai-founder-became-a-billionaire-by-building-chatgpt-for-doctors/">OpenEvidence kasutajate seas on suur protsent USA arstidest</a>. Kui ma oma healt s&#245;bralt k&#252;sisin, kuidas tema seda &#252;ldkirurgina kasutab, &#252;tles ta, et dilemma k&#252;simusi ikka &#228;ra ei lahenda, aga sellisele alustavale &#252;ldarstile kindlasti hea t&#246;&#246;riist. <a href="https://www.youtube.com/watch?app=desktop&amp;v=cOTDV0_CuWs">Sotsiaalministeeriumi &#252;ritusel</a> r&#228;&#228;kisid ka Eesti arstid, kuidas nad ChatGPT igap&#228;evat&#246;&#246;s kasutavad.</p><p>Hiljuti j&#228;i silma ka artikkel, et <a href="https://www.worksinprogress.news/p/why-ai-isnt-replacing-radiologists/comments#comment-159769499">AI ei hakka niipea radiolooge asendama</a> ja see on valdkond, mida ennustati juba ammu esimeseks AI automatiseerimise ohvriks. Tuleb v&#228;lja, et radiolooge on aina enam vaja, sest muuhulgas teeb AI t&#246;&#246;d efektiivsemaks, sh ka eestlaste <a href="https://bettermedicine.ai/">BetterMedicine</a>. Klassikaline <a href="https://et.wikipedia.org/wiki/Jevonsi_paradoks">Jevonsi paradoksi</a>.</p><p>Eestil on siin muidugi tohutu v&#245;imalus: meil on digitaalsel kujul olemas kogu rahvastiku terviseandmed JA meil on ka rahva geeniandmed olemas. Ehk teoorias <strong>suudaks riik ehitada ennetava meditsiini superlahenduse</strong>, mis v&#245;taks aluseks k&#245;igi inimeste k&#245;ik terviseandmed ning suunaks neid positiivseid samme (<em>pun intended</em>) tegema: alates sellest, et mida s&#252;&#252;a v&#245;i kas teha zone-2 v&#245;i j&#245;utrenni, l&#245;petades sellega, millal neerude veremarkereid &#252;le kontrollida ja eesn&#228;rme kontrolli minna. L&#252;hemad j&#228;rjekorrad (pere)arstidel, varem avastatud haigused ja sellega seoses v&#228;iksem kulu tervishoiule. Nortalil on sarnase idee kohta ka hea <a href="https://www.personalgovernment.com/personal-healthcare">n&#228;ide</a> ja <em><a href="https://www.personalgovernment.com/personal-healthcare-white-paper">white paper</a></em>.</p><p>Kui keegi selle Excel&#8217;isse paneks, tuleks t&#245;en&#228;oliselt v&#228;lja, et selline lahendus oleks <em>no-brainer</em> rahva tervise ja kuluefektiivsuse vaates. Hiljuti &#252;hel kinnisel &#252;marlaual k&#228;ies tunnistas &#252;ks meie tipp-poliitikuid, et t&#245;esti, rahvastiku vananemise probleem on nii j&#245;hker, et kellelgi pole &#245;rna aimugi, kuidas riik tulevikus pensioni- ja tervishoiukohustusi lahendama hakkab.</p><h2>Ja edasi?</h2><p>On veel mitmeid teemasid, milleni ma j&#245;udnud ei ole, aga on l&#228;hiaja plaanides. &#220;ks on kindlasti <strong>lastele AI projektide tegemine</strong>, sest ka nende kohta tekib omajagu k&#252;simusi: alates vasts&#252;ndinu arenguh&#252;petest l&#245;petades 5-aastase k&#252;lmetuse raviga. Apple Health (kell, kaal jm) ja Oura s&#245;rmus v&#245;iksid oma andmeid jooksvalt AI-le edastada, et ta saaks &#252;kshetk proaktiivselt soovitusi hakata jagama. Selleks tuli hiljuti ChatGPT-l ka <a href="https://www.youtube.com/watch?v=nk6IjAnCCqw">Pulse funktsioon</a> v&#228;lja. Andmete edastamiseks on aga vaja <em>vibe code</em>&#8216;ida (ehk lasta AI-l programmeerida) lihtne pudin, mis t&#245;mbaks andmeid &#252;hest API-st ja s&#246;&#246;daks neid mu AI-le. Samas ei saa mainimata j&#228;tta, et elektriautod ei lahenda linnades autostumise probeemi ehk nii liikuvuse, m&#252;ra, turvalisuse kui ka kliima osas on t&#228;htis edendada samaaegselt jalak&#228;ias&#245;bralikke linnu, edendada rattataristut ja investeerida &#252;histransporti.</p><p>Hankisin 2021. a detsembris kasutatud Tesla Model 3 Long Range'i ja seega on mu kogemus piiratud Teslaga, kuigi laieneb paljuski ka teistele elektriautodele.</p><p>Head:</p><ul><li><p>Kodus laadimine on &#228;&#228;rmiselt mugav ja minu jaoks &#252;ks suurimaid plusse. Mulle &#252;ldse ei meeldinud tanklas k&#228;imine ja n&#252;&#252;d toimub u 90% laadimistest hoopis kodus<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-3" href="#footnote-3" target="_self">3</a>. Kojuj&#245;udes panen auto alati seina ning eestlaste tehtud <a href="https://www.gridio.io/">Gridio</a> tarkvara laeb autot ise soodsaima b&#246;rsihinna ajal.</p></li><li><p>Tarkvara on veel teema, mida minu hinnangul enamus autode puhul alahindab &#8212; auto on nutitelefon ratastel. Paljud uued autod (sh kallid preemium autod) on tarkvaralt 2006 aasta Nokia tasemel<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-4" href="#footnote-4" target="_self">4</a>. See on koht, kus Tesla hiilgab oma v&#228;ga hea tarkvaraga, mis uueneb iga paari n&#228;dala tagant. 9 kuuga on auto t&#228;nu uuendustele m&#228;rgatavalt mugavamaks ja paremaks muutunud.</p></li><li><p>S&#245;idukogemus on fantastiline ja see on kindlasti parim auto, millega ma s&#245;itnud olen. T&#228;nu 4-veole saab ka hakkama talvel Otep&#228;&#228;l kinnituisanud teedel s&#245;itmisega ja &#252;le lumevallide ronimisega.</p></li><li><p>Elektriautode s&#245;idu teeb veel eriliseks nn &#252;he pedaaliga s&#245;itmine ehk gaasipedaalilt jala v&#245;ttes, peatub auto vaikselt ning laeb samal ajal akut (nn <em>regenerative breaking</em>). See n&#245;uab harjumist, kuid l&#245;pus on <em>nobrainer</em> ning s&#228;&#228;stab ka pidurikettaid, sest kohati on s&#245;ite-p&#228;evi, kus pidureid ei kasutagi.</p></li><li><p>M&#246;&#246;das&#245;itudel jm ei j&#228;&#228; &#252;ldse h&#228;tta, sest kuigi tegemist on pereautoga, siis kiirendus on sportautodega samal tasemel.</p></li><li><p>Kuigi auto alghind on kallim, on muud kulud soodsamad: madalam liisinguintress, v&#228;iksem hooldus- (elektriautos suurusj&#228;rgus 10 korda v&#228;hem juppe kui tavaautos) ja laadimiskulu. Kodus Gridio'ga laadides v&#245;ib kohati t&#228;isaku (400-500km) saada m&#245;ne euroga, sest elektrimootorid on &#228;&#228;rmiselt efektiivsed (<a href="https://ecocostsavings.com/electric-car-kwh-per-mile-list/">M3LR on n&#228;iteks 0,16 kWh/km</a>). Et elektri hind v&#245;rdsustuks bensiini-diisliga peaks laadima suurusj&#228;rgus 600&#8364;/MWh hinnaga, mis on r&#246;&#246;gatult k&#245;rge.</p></li><li><p>Ma pole kunagi olnud <a href="https://twitter.com/davidzipper/status/1496103677127446541?s=21">eriline valjude autode f&#228;nn</a>, seega naudin helitut autot. Linnas on sellel pikaajaliselt ka positiivne m&#245;ju m&#252;rale, sest alla 30-40kmh on elektrikad oluliselt vaiksemad. See on isegi kummaline, kui paljud tuttavad on k&#252;sinud, kas pean 4 aasta p&#228;rast aku v&#228;lja vahetama. Tesla n&#228;itel on aku ja mootorite garantii&nbsp; 8 aastat v&#245;i 192 000 km (oleneb, kumb enne k&#228;tte j&#245;uab) ning s&#245;idusolevates autodest on 300 000km l&#228;bis&#245;idu puhul on akust u 90% mahtu alles. V&#228;henemist on, kuid see on v&#228;ike.</p><p>Vead:</p><ul><li><p>Kuigi talvel s&#245;idab Tallinnast Otep&#228;&#228;le &#228;ra, v&#228;heneb tugevate miinuskraadide ja tuulega <em>range</em> oluliselt. Kogemuse p&#245;hjal saab tavap&#228;rasest 400-500km'st 300-400km. Praktikas ei viitsi ma (ega mu p&#245;is) pea kunagi &#252;le 200km j&#228;rjest s&#245;ita.</p></li><li><p>Alghind on elektriautodel ikka k&#245;rgem, kuid see v&#228;heneb vaikselt ajas ning m&#245;ne aasta p&#228;rast peaks elektriauto muutuma soodsamaks. Samas on k&#245;rge alghinna &#252;ks p&#245;hjustest arvestatav lisavarustus, mis on hinna sees &#8212; klaaskatus, kaamerad, p&#252;sikiiruse ja s&#245;iduraja hoidja, istmesoojendused ka taga jne, mis on tavaautodel lisahinna eest (nt BMW tuleb sarnase lisavarustuse lisamisel sisuliselt sama hinnaga nagu Tesla).</p></li><li><p>Tesla puhul on kindlasti suurim miinus see, et teenindus ainult Helsingis. Positiivse k&#252;lje pealt tahavad elektrikad j&#228;llegi v&#228;hem hooldust, kuid siiski on see p&#245;hjus, miks mitmed eelistavad Eesti esindusega automarki.</p></li><li><p>Elektriautode raskuse t&#245;ttu on nendel ka suurem rehvide kulumisest tekkiv saaste. <a href="https://www.theguardian.com/environment/2020/jul/14/car-tyres-are-major-source-of-ocean-microplastics-study?CMP=share_btn_tw">Rehvipuru j&#245;uab omakorda veekogudesse olles oluline mikroplastiku allikas</a>.</p></li></ul><h2>III</h2><p>Elektriautodest ei saa r&#228;&#228;kida, kui ei puuduta ka laadimistaristut. Nagu ka paljude teiste rohep&#246;&#246;rde teemadega, on Eesti j&#228;lle maas: maailmas on <a href="https://cleantechnica.com/2022/08/28/electric-vehicles-10-of-new-vehicle-sales-globally/">10% uutest autodest t&#228;iselektrilised</a>, <a href="https://amtel.ee/augustis-oli-uute-soiduautode-turg-tanavu-esimest-korda-plussis/">Eestis ainult 3%</a><a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-5" href="#footnote-5" target="_self">5</a>. Kui tuleb rohkem elektrikaid, paraneb ka laadimisv&#245;rk. V&#245;i oli see vastupidi?</p><p>Isikliku kogemuse p&#245;hjal laen ma kodus 90% ajast, kontoris 5% ja ja v&#228;ljas 5%. Eestis on avalik laadimisv&#245;rk n&#245;rgal tasemel ehk arenguruumi on veel k&#245;vasti: mitmetes olulistes sihtkohtades pole veel &#252;htegi laadijat, kuid see muutub juba ainu&#252;ksi <a href="https://www.riigiteataja.ee/akt/130062020009">regulatsioonide</a> t&#245;ttu peagi. Ka on jabur, et laadimiseks tuleb kasutada operaatorite mobiili&#228;ppe, millest osasid kirjeldab k&#245;ige paremini s&#245;na "n&#228;ss".</p><p>Astri Grupi poolt n&#228;eme, et avalike laadijate rajamise huvi on v&#228;ga suur. Ainu&#252;ksi meie keskuste ja turgude juures kahekordistub j&#228;rgneva 6 kuu jooksul laadijate arv ning uutest operaatoritest on pikk j&#228;rjekord ukse taga. Lisaks muud v&#228;iksemad venitused, sinikad.</p><p>Seoses konkreetse kohaga, Pirita ja Tallinnaga laiemalt tekkisid m&#245;ned ettepanekud linnaplaneerijatele:</p><ol><li><p>T&#228;histada liiklusm&#228;rkidega k&#228;idavamad &#252;lek&#228;igud, eriti mis on kooli, lasteaedade, hoidude jm juures. Lisada ka LED valgustus. Tegelikult v&#245;iksid uued &#252;lek&#228;igud arvestada ka vaegliiklejatega.</p></li><li><p>Lahendada &#252;lek&#228;igud t&#245;stetud ristmikuna, mis on n&#228;iteks Pirita keskuse ees juba olemas.
